COVID-19 Vaccine Assistance – South End Senior Center

Unfortunately, the healthcare provider has indicated that due to this you will need to wait 2 weeks to receive your vaccine. We ask that you call back then to schedule an appointment.

Agent Instructions:
1. Ask caller if they received first dose at this same location. If no, refer caller to try and book appointment at the location where they received first dose.
If they booked first appointment through us at the Assist Line and the site did not book their second appt for them while they were at their first appt, then continue.
2. Navigate to Patients tab in Appt Plus (
www.appointmentplus.com
) to search for the caller and see if they already have an appt scheduled for their second dose. If not, proceed with booking second dose appointment -- ask if Pfizer (book as close to 21 days, but not earlier, from their first appt) or Moderna (books as close to 28 days, but not earlier, from their first appt).

Search for client (by last name or phone number) to see if the client already exists in Appt Plus.
2. If caller already has a profile in Appt Plus, check to ensure they don’t already have a first dose appointment scheduled. If they do, ask them if they can keep that existing appointment or if they want to cancel it.
3. If caller does not have a profile in Appt Plus, navigate back to the calendar to search for the first available appointment that is
at least 1 full business day in advance

4. Once a date/time is found, click the (+) to complete the patient profile info and book the appointment.
BOOKING RULES:
*No booking same-day appointments

next-day business appointments (see guide above).
*First dose appointments cannot be booked more than 20 days in the future.
IMPORTANT
: Please read the spelling of the caller's first name, last name and date of birth back to the caller to ensure accuracy. All patient details must be correct.
CLOSING SCRIPT:
Your appointment is booked through the affiliation with Trinity Health of New England and The City of Hartford at the South End Senior Center. Locations is: 830 Maple Avenue, Hartford, CT 06114. This is not a drive-thru site, so you will need to be able to get out of your vehicle and walk into the building. The building is handicapped accessible.
You’ll receive an email/text confirmation for this appointment if you have an active email address or can receive text messages.
Please arrive at your scheduled appointment time and be prepared to remain on site after the vaccine for 15-30 minutes to ensure your safety. During the observation time after you receive the vaccine, someone will make you an appointment for your 2nd dose.
We recommend you wear a short sleeve shirt that gives us easy access to your upper arm, just below the shoulder.
For the safety of you and others please do not come to our vaccine clinic if you are feeling ill anytime in the 24 hours before your appointment.
Please remember to bring your insurance card with you. If you do not have an insurance card, that's okay, it will not prevent you from getting the vaccine.
Remember, this appointment is only for you – we are not accepting patients without an appointment. If you have friends/family who are interested and eligible to receive the vaccine, please have them call us at 1-877-918-2224.
