COVID-19 Vaccine Assistance – Danbury Health Dept

I am afraid we can not book an appointment without sharing your information with the provider.
Unfortunately, the healthcare provider has indicated that it is not advised for you to receive the vaccine at this time. They are asking that you wait two weeks and then call back to schedule an appointment.
Okay, let’s see about getting your appointment booked. Is this an appointment for your first dose?

Agent Instructions:
1. Ask caller if they received first dose at this same location. If no, refer caller to try and book appointment at the location where they received first dose.
If yes, ask whether they booked their first appointment through VAMS or through us at the CT Vaccine Appointment Assist Line.
If they booked first appointment through VAMS, they need to try and book second appointment through VAMS.
If they booked first appointment through us at the Assist Line and the site did not book their second appt for them while they were at their first appt, then continue.
2. Navigate to Patients tab in Appt Plus (

) to search for the caller and see if they already have an appt scheduled for their second dose. If not, proceed with booking second dose appointment -- ask if Pfizer (book as close to 21 days, but not earlier, from their first appt) or Moderna (books as close to 28 days, but not earlier, from their first appt).
Agent Instructions:
1. Navigate to Patients tab in Appt Plus

Search for client (by last name or phone number) to see if the client already exists in Appt Plus.
2. If caller already has a profile in Appt Plus, check to ensure they don’t already have a first dose appointment scheduled. If they do, ask them if they can keep that existing appointment or if they want to cancel it.
3. If caller does not have a profile in Appt Plus, navigate back to the calendar to search for the first available appointment that will work for the caller. Once a date/time is found, click the (+) to complete the patient profile info and book the appointment. **First dose appointments cannot be booked more than 20 days in the future. Appointments for the next day cannot be made after 6pm EST.

IMPORTANT
: Please read the spelling of the caller's first name, last name and date of birth back to the caller to ensure accuracy. All patient details must be correct.
CLOSING SCRIPT:
Your appointment is booked with the Danbury Health Department at the Rogers Park Middle School at 21 Memorial Drive in Danbury. The best way to access the clinic is through the driveway off of Mountainville Avenue, next to the Rogers Park Middle School baseball field. This is not a drive-thru site, so you will need to be able to get our of your vehicle and walk into the building.
You’ll receive an email/text confirmation for this appointment if you have an active email address or can receive text messages.
Please arrive at your scheduled appointment time and be prepared to remain on site after the vaccine for 15-30 minutes to ensure your safety. During the observation time after you receive the vaccine, someone will make you an appointment for your 2nd dose.
Remember, this appointment is only for you – we are not accepting patients without an appointment. If you have friends/family who are interested and eligible to receive the vaccine, please have them call us at 1-877-918-2224.
