COVID-19 Vaccine Assistance – Community Health Center – Middletown

Unfortunately, the healthcare provider has indicated that due to this you will need to wait 2 weeks to receive your vaccine. We ask that you call back then to schedule an appointment.

Agent Instructions:
1. Ask caller if they received first dose at this same location. If no, refer caller to try and book appointment at the location where they received first dose. If yes, continue.
2. Navigate to Patients tab in Appt Plus (
www.appointmentplus.com
) to search for the caller and see if they already have an appt scheduled for their second dose. If not, proceed with booking second dose appointment 21-24 days after their first dose appointment.

Search for client (by last name or phone number) to see if the client already exists in Appt Plus.
2. If caller already has a profile in Appt Plus, check to ensure they don’t already have a first dose appointment scheduled. If they do, ask them if they can keep that existing appointment or if they want to cancel it.
3. If caller does not have a profile in Appt Plus, navigate back to the calendar to search for the first available appointment that will work for the caller. Once a date/time is found, click the (+) to complete the patient profile info and book the appointment. **First dose appointments cannot be booked more than 18 days in the future. Appointments for the next day cannot be made after 6pm EST.

IMPORTANT
: Please read the spelling of the caller's first name, last name and date of birth back to the caller to ensure accuracy. All patient details must be correct.
4. Navigate to the Patients tab in Appt Plus to search for the client, open the client profile, and click on Make Appointment to proceed with booking the second dose appointment. ** The second dose needs to ideally be 21 days after the 1st dose. If caller cannot make an appointment on day 21, you may book appointment up to day 24 but not before day 21. Use the
21 day calculator
to add 21 days to the first dose date to calculate the correct date for the second dose appointment.
CLOSING SCRIPT:
Your appointment is booked with Community Health Center in Middletown at the Wesleyan Vine Street Parking Lot, which is also known as the John Wood Memorial Tennis Courts parking lot. This parking lot is at 75 Vine Street, which is at the intersection of Vine Street and Knowles Avenue. And please remember this is a drive thru site and you must be in a vehicle.
You’ll receive an email/text confirmation for both of these appointments if you have an active email address or can receive text messages.
Please
do not drive up before
your appointment time. It is very important that you do not arrive early or late, but as close as possible to your exact appointment time. When you arrive at the site, you will need to complete a short clinical survey and will need to remain on site after the vaccine for 15-30 minutes for both appointments to ensure your safety.
We recommend you wear a short sleeve shirt that gives us easy access to your upper arm, just below the shoulder. We recommend you remove all outer jackets prior to arriving to the vaccine area to expedite the process.
If you are being driven to the site, please sit in the rear seat on the driver side so that the provider can vaccinate the left arm, unless you have a need for the right arm to be vaccinated.
Please remember to bring your insurance card with you. If you do not have an insurance card, that's okay, it will not prevent you from getting the vaccine.
Remember, this appointment is only for you – we are not accepting patients without an appointment. If you have friends/family who are interested and eligible to receive the vaccine, please have them call us at 1-877-918-2224.
